Patient Responsibilities

  • To treat doctors and all practice staff with courtesy and respect.
  • Please let us know if you are unable to keep an appointment.
  • Appointments are for one person - if two people need to see the doctor you need to make two appointments
  • Arrive on time for your appointments - or let us know if you are going to be late. This may lead to a shorter appointment.
  • Attend for monitoring when the doctor has advised this is necessary.
  • Request a home visit only if absolutely necessary.
  • You are responsible for your own health and that of your children. Please take the advice that is given to you at the practice. Let us know immediately if you change your address, name or telephone number. In case of routine referral for a hospital appointment, if an appointment is not received within six weeks, contact the hospital concerned.
